Targeted Case Management

The BRANCH Program incorporates mental health targeted case management services (MHTCM) to help participants gain access to necessary medical, social, educational, and other services and supports. MHTCM activities include:

Case managers work as part of the participant's therapeutic team and maintain communication with all involved service providers. They serve as advocates for participants while empowering them to eventually manage their own care and services. The frequency and intensity of case management is determined by individual participant needs.

    • Comprehensive initial assessment and periodic reassessment of participant needs

    • Development and regular revision of trauma-informed, person-centered care plans

    • Setting specific goals and actions to address needed services

    • Working collaboratively with participants and/or caregivers to develop goals

    • Making referrals and scheduling appointments to help obtain needed services

    • Linking participants with medical, social, behavioral, and educational providers

    • Coordinating with other programs and services to meet identified needs

    • Actively assisting participants in accessing necessary resources

    • Regular monitoring to ensure services are being delivered as planned

    • Evaluating if services are adequate in amount, scope, and duration
